1. In the Environmental Compliance > Fats, Oils, Grease > Facilities > Facilities module, click the button.
2. Enter a Facility Number (required). This value must be unique.
3. Create a Facility Name to further identify the entity.
4. Enter the address of the facility.
5. Indicate whether this is currently an Active Facility in the FOG program.
6. Complete other fields as desired.
7. Click the button to save the record. The new record appears in the FOG Facility grid.

Method 1: Add a Grease Extractor within the FOG Facility Module

1. In the FOG Facilities module, click the button to expand the Facility to which you want to add an extractor.
2. Select the FOG Grease Extractor grid and click the button. The system creates a new Grease Extractor record with the selected Facility's ID in the Facility Number field.
3. Continue to Filling out the Record (below).

Method 2: Create a Record in the FOG Grease Extractors Module

1. Open the Fats, Oils, Grease > Facilities > Grease Extractor module.
2. Click the button. A new record appears.
3. Enter or select a Facility Number (required).
4. Continue to Filling out the Record (below).

Filling Out the Record

1. Enter a unique Unit ID (required) for the extraction equipment.
2. If desired, provide a brief Description that distinguishes this item from other grease extraction equipment at the facility.
3. Describe the item's General Location.
4. Complete the attribute fields as desired.
5. Click the button to save the record. The new record appears in the FOG Grease Extractors grid.

Method 1: Add an Inspection Record from the FOG Facility Module

1. In the FOG Facilities module, click the button to expand the FOG Facility record to which you want to add an inspection.
2. Select the Facility Inspection grid and click the button. The system creates a new Inspection record with the selected Facility's ID in the Facility Number field.
3. Continue to Filling Out the Record (below).

Method 2: Add an Inspection Record from the FOG Facility Inspections Module

1. Open the Fats, Oils, Grease > Facilities > Inspection module.
2. Click the button. A new record appears.
3. Enter or select a Facility Number (required).
4. Continue to Filling out the Record (below).

Filling Out the Record

1. Enter the Inspection Date (required).
   • The combination of Facility Number and Inspection Date must form a unique combination.
2. Complete other fields as desired.
3. Use the Maintenance Requested or Needs Repair field to automatically create a Request or Work Order for the site, if desired.
4. Click the button to save the record. The new Inspection record appears in the grid.

Method 1: Add an Event from the FOG Facility Module

1. Open the Fats, Oils, Grease > Facilities module.
2. Click the button to expand the Facility record to which you want to add an event.
3. Select the Facility Events grid and click the button. The system creates a new Facility Event record with the selected Facility's ID in the Facility Number field.

Method 2: Create a Record in the FOG Facility Events Module

1. Open the Fats, Oils, Grease > Facilities > Facility Events module.
2. Click the button. A new record appears.
3. Enter or select a Facility Number (required).
4. Continue to Filling out the Record (below).

Filling Out the Record

1. Enter or select the Logged Date (required).
2. Select a Group from the pick-list (required). These options come from the Fats, Oils, Grease > Facilities > Setup > Group Setup module.
3. Choose an Event type from the pick-list. (The choice the user makes in the Group field determines the options in the Event field.)
4. Complete other fields as desired.
5. Click the button to save the record. The new Facility Event record appears in the grid.

1. Click the button to expand the record.
2. Select the Contact grid and click the button. The Contact form opens.
3. Enter the contact's Name. (This is required and must be unique for this record.)
4. Complete the other fields as desired.
5. Click the button to save the Contact. The new entry now appears in the Contact grid.

Method One: Add a Record through the FOG Facility's Notification History Grid

1. Click the button to expand the FOG Facility record to which you want to add a Notification.
2. Select the Notification History grid and click the button.
3. Specify the type of Notification (required).
4. Enter the Date Sent. (This is required and must be unique; therefore, users may only record one Notification per Facility per day).
5. Complete the other fields as desired.
6. Click the button to save the Notification. The new Notification appears in the grid.

 
Method Two: Use the Notification Fields on the FOG Facility Form

1. Click the button next to the desired FOG Facility record. The Sewer FOG_Facility Form appears.
2. Locate the Notification field and use the pick-list to select the type of notification being sent.
3. Enter the Notification Date.

   Note: If a user enters a value for either the Notification or Notification Date field, the other field will be required, as well.

4. Save and Close the record. A corresponding record now appears in the Facility's Notification History grid. The system creates a new History record each time the Notification/Notification Date fields are modified on the Facility form.

1. Click the button next to the FOG Facility record to which you want to add a conversation.
2. Select the Conversations grid and click the button. The Conversations form appears.
3. Enter the Contact.
4. Select the Agency Rep.
5. Enter the Conversation Date/Conversation Time (required).
6. Specify the Contact Direction and Conversation Type.
7. Enter the contact's Phone Number.
8. Provide a Short Description to summarize the nature of the conversation.
9. Record pertinent details in the General Comment field.
10. Click the button to save the Conversation. A new record now appears in the Conversations grid.

    Note: Two read-only fields, Date Recorded and Time Recorded, appear at the bottom of the form. When the record is saved, the system uses these fields to record the date and time the conversation record was created.

1. Click the button next to the FOG Facility record to which you want to add a maintenance log.
2. Select the Maintenance Logs grid and click the button. The Maintenance Logs form appears.
3. Select the Log Type (required) from the pick-list.
4. Enter the Submitted Log Date or press the space bar with the cursor in the field to automatically enter today's date.
5. Enter the Log Freq (number) and select the Log Freq Units. The system automatically calculates the Next Log Due Date (required).
   • Or, simply manually enter the Next Log Due Date.

     Note: The Log Type and Next Log Due Date fields are required and must form a unique combination.

6. Enter a Short Comment about the maintenance activity, if desired.
7. Click the button to save the data. A new record now appears in the Maintenance Log grid.

1. Click the button to expand the Facility or Facility Inspection record to which you want to add a notice or violation.
2. Select the Notices and Violations grid and click the button. The Notices and Violations form appears.
3. Use the pick-list to select an Enforcement Type.
4. Enter the Occurred Date (required).
5. Choose the specific Notice/Violation (required) that applies. (These pick-list options are carried over from the Violation Setup module.)
   • The Occurred Date and Notice/Violation fields are required and must form a unique combination with the parent record.
6. Complete other fields as desired.
7. Click the button to save the record. The new Notice or Violation now appears in the grid.

1. Select the Violations (or Notices and Violations) tab.
2. Using the button, expand the Violation record to which you want to add a Corrective Action.
3. Select the Corrective Actions grid and click the button. The Corrective Action form opens.
4. Use the pick-list to select a Corrective Action and enter the Notification Date.
   • These fields are required and must form a unique combination for the selected Violation.
5. Complete other fields as desired.
6. Click the button to save the record. The Correction Action now appears in the grid.

1. Click the button to expand the Facility record to which you want to add a SIC or NAICS code.
2. Select the SIC Codes grid OR the NAICS Codes grid and click the button.
3. Use the pick-list to select the code designated for the business activity that takes place at the facility.
4. Click the button to save the record. The new Code now appears in the grid.
