Request Submittal forms are used for entering work requests. These can be available on both the internal Web application and the Web Citizen application. Depending on where they are accessed these forms will appear differently.
Special Checkbox
Remember Me - Used on the form to keep the requester's name and contact information saved in a cookie. The next time the user logs in this information will be autofilled.
Request Lookup

To look up requests with similar information click this button after filling out the information in the field preceding it. A popup will appear with all requests (opened and closed) that match that field.
Select a request from the list and click ok to open a new view with that request. Users can then create a request from the previous existing request. This will link the two requests together. |
CAPTCHA
CAPTCHA can be used on the Citizen Request forms to help reduce spam. If this option has been implemented, a code similar to the following will appear on the request web page. Users will be required to type the code prior to submitting their request.
Knowledge Base
Based on the Work Flow Setup, some Problems may have associated Knowledge Base information when selected on a Request submittal form. This is designed as a popup to provide a user more information about a problem
