Sample Request Form

Below, we've provided an example of a Citizen Request form. This is an example of a Detail Form that is not displayed in grid format. This particular custom form requires users to enter their contact information and type the details of their request in a comment grid. The "Remember Me" checkbox at the top of the form allows the system to remember users' contact information. This will allow users to quickly enter future requests. Keep in mind that all forms can be customized by the system administrator. The fields and their arrangement on the form will vary.
