Sample Facility Request

Below, we've provided an example of a Facility Request form. This custom form requires users to select a Facility Type, particular facility, and problem from three separate pick lists. In addition, this form has an area for user comments to explain the problem. There's a date field to indicate when the problem needs to be fixed, and finally, an email field to enter the user's contact information.
