This tab allows you to record detailed information about the reported illicit discharge. You can record the primary location of the problem (i.e. in or near stream, on land away from stream) and the secondary location of the problem (i.e. pipe, in stream, along banks, etc.). In addition, a text field is provided to describe the problem location.
Numerous check-boxes allow you to indicate the presence or absence of specific problem attributes in or near the stream. These include the presence and type of odor, the stream's physical appearance, and the presence and type of floatables. Check-boxes are also provided to indicate the presence or absence of dumping, oil/solvents, sewage, wash water, and other problems on land away from the stream.
Finally, two text fields are provided allowing you to describe the pollution and suspected violator(s).
